*Key Responsibilities - Support Worker *
Provide high-quality care and support to individuals with various disabilities, ensuring their physical, emotional, and social needs are met.
Assist with daily living activities such as personal care, meal preparation, and medication administration.
Facilitate social inclusion by encouraging participation in community activities, educational programs, and recreational events.
Collaborate with the care team to develop, implement, and review individualized support plans for each service user.
Maintain accurate records and reports of activities, observations, and any changes in the health or behavior of the service users.
Promote independence by encouraging and supporting self-care skills and daily living tasks.
Respond effectively to emergencies and follow established procedures during critical incidents.
Contribute to a positive working environment by demonstrating respect, empathy, and professionalism towards colleagues, service users, and their families.
Collaborate with other agencies and professionals involved in the care of service users to ensure coordinated and comprehensive support.
Ensure that the service user's rights are respected and promote choices, empowerment, and dignity at all times.
Participate in ongoing training and professional development opportunities to expand knowledge and skills related to supporting individuals with disabilities.
Contribute to a safe working environment by following health and safety policies and procedures, maintaining confidentiality, and adhering to relevant legislation and regulations.
Ensure that the service user's living spaces are clean, comfortable, and well-maintained at all times.
Act as a keyworker for assigned service users, providing ongoing support, guidance, and emotional care.
Work collaboratively with families, carers, and other stakeholders to maintain positive relationships and ensure effective communication about the service user's needs and progress.
